This section of our website is provided to help individual donor advisors facilitate the processing of all forms of proceeds for events.

 All forms in this section are geared to help our funds with the reporting of all proceeds in a very timely and efficient manner and should be used by all to submit gifts for processing.  There are Excel spreadsheets for forms that can be filled in on the computer and there are forms for those whom choose to fill them in by hand.
